Per Diem Pharmacist - Brooklyn Hospital Center : Job Details

Reviews and interprets medication orders, evaluates the patients drug regimen relative to the disease state, allergies, drug and food interactions and pertinent laboratory data prior to preparation and dispensing. Interacts with prescribers to ensure accuracy, completeness and appropriateness of prescribing or ordering across all age groups represented by the population served. Monitors drug therapy in conjunction with prescribers, nurses and other healthcare professionals to promote safe, effective therapeutic regimens. Supervises activities of Pharmacy Interns, technicians and students. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Bachelors (BS) or Doctor of Pharmacy (PharmD.) Degree Program.
