**Preferred Home Care of New York,** a fully licensed Home Care Agency serving the NY Metropolitan area and beyond, is seeking Licensed Practical Nurses to service our concierge patients in Brooklyn, Queens, and the Bronx each nurse will have the opportunity to service patients within one borough, and will not have to travel beyond that borough. This position requires expertise in the areas of patient and family assessment, determining appropriateness of and acceptance for home health services, coordination of services given by other health care providers, and knowledge of state regulations. The Licensed Practical Nurse will enjoy being a part of a dynamic, supportive, and compassionate team of healthcare professionals.

**Requirements:**

* Must have an active New York State LPN License

* Strong verbal and written communication skills

* Knowledge and understanding of Home Care policies and procedures

* Preferred but not required experience working in an Adult care facility & Assistant Living Program

* UAS Experience preferred
