Position: Pediatric Home Health RN

Scope:

The Pediatric Home Health RN specializes in providing comprehensive nursing care to pediatric patients in home settings. They assess, plan, implement, and evaluate care tailored to the unique needs of pediatric patients and their families.

Qualifications:

Graduate of an accredited nursing program.

Current RN licensure in New York.

Prior pediatric nursing experience preferred, with a focus on home health highly desirable.

Knowledge of pediatric developmental stages and family-centered care principles.

Excellent communication and interpersonal skills.

Duties:

Conduct thorough assessments of pediatric patients, including physical, developmental, and psychosocial assessments.

Collaborate with pediatric patients, their families, and interdisciplinary team members to develop individualized care plans.

Provide skilled nursing care to pediatric patients, including but not limited to medication administration, wound care, tracheostomy care, ventilator management, and tube feedings.

Monitor pediatric patients' health status and response to treatments, interventions, and medications.

Educate pediatric patients and their families on disease management, medication administration, safety precautions, and health promotion.

Coordinate with physicians, therapists, social workers, and other healthcare professionals involved in the care of pediatric patients.

Document all patient assessments, interventions, and evaluations accurately and timely.

Participate in ongoing education and training related to pediatric home health care.

Adhere to infection control protocols and safety standards to ensure the well-being of pediatric patients and their families.

Provide emotional support and counseling to pediatric patients and their families, promoting a supportive and nurturing environment.

Advocate for the needs and rights of pediatric patients and their families within the healthcare system.
