Position Summary: Under the direct supervision of the Teen Clinic (TC) Program Supervisor, the TC Patient Services Representative (PSR)/ Medical Assistant (MA) will perform patient registration, provide information on FPACT, enroll eligible patients under FPACT, answer patient inquires, as well as research and resolve customer problems. General responsibilities will be, but are not limited to, developing teen appropriate health education presentations & materials, training youth and delivering health education, referring youth to SYHC Network of Care, and other specified health education duties as assigned. Essential Functions of the Job: Essential Duties (PSR)
  • Greet patients as they enter Teen Clinic or by telephone when patient calls come in.
  • Schedule appointments in person or via Facebook or other appointment scheduling systems.
  • Process new and existing patients for their appointment or Walk-In visits.
  • Assemble all data and documents required to complete patient registration, such as personal and demographic information.
    • Retrieve and enter patient's registration information on EPIC system, confirm and verify patient's information.
    • Determine FPACT eligibility and enroll eligible individuals, upload/submit verification documents and completing applications.
  • Assist with redetermination/renewal process for patients.
  • Work in collaboration with all areas of patient inquiry to identify and resolve issues and/or barriers.
  • Respond to requests for information and inquiries related to patient access processes, policies, and/or other related information; research and resolve customer problems.
  • Recognize the need for immediate and urgent services.
  • Fill out and be responsible for keeping logs and numbers of all outreach activities that contribute to Teen Clinic numbers.
  • Attend and provide program information at multiple Health Fairs and Festivals in the target communities.
  • Attend Community Meetings as requested by Supervisor.
  • Report and meet with the Supervisor as needed.
  • Conduct activities that will contribute promotion of healthy lifestyles.
  • Conduct self in a professional and positive manner in all clinic and community related activities.
  • Provide assistance, guidance and direction to visitors and patients; calls for an escort when necessary.
  • Assist in care and maintenance of department equipment and supplies.
Essential Duties (MA)
  • Take vitals for teen clinic patients including blood pressure, temperature checks, height, weight, etc.
  • Administer and review the teen health history form and review for completeness.
  • Collect required samples, conduct pregnancy test, and prepare Labs.
  • Complete required MMR for positive STD's and submit to County.
  • Document patient information into EPIC, including tracking labs and assigning tasks to providers as appropriate.
  • Work with the main clinic Registered Nurse to order medical supplies as needed. Ensure appropriate stock of items for clinic.
  • Work in collaboration with main clinic MA's when appropriate to ensure patient access and care.
  • Report and meet with the Supervisor as needed.
  • Conduct self in a professional and positive manner in all clinic and community related activities.
  • Provide assistance, guidance and direction to visitors and patients; calls for an escort when necessary.
  • Assist in care and maintenance of department equipment and supplies.
Additional Duties and Responsibilities:
  • Maintain established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Maintain program specific records as required; submit reports, including activity, contact and mileage reports in accordance with agency policies and procedures.
  • Attend meetings and/or training sessions as required and participates on committees as directed.
  • Perform other related duties as assigned.
Job RequirementsExperience Required:
  • One-year of experience as a receptionist or other customer service field.
  • Previous experience in a medical setting or
  • Previous experience as Medical Assistant (entry level is fine).
Education Required:
  • High School Diploma or GED equivalent
  • Medical Assistant Certification
Verbal and Written Skills Required to perform the Job:
  • Familiarity with community resources and their use to meet the needs of families.
  • Demonstrates excellent oral and written communication skills.
  • Has organization and project management skills.
  • Must maintain confidentiality at all levels.
  • Bilingual in Spanish preferred
Technical Knowledge and Skills Required to Perform the Job:
  • Ability to establish and maintain good interpersonal relationships.
  • Proven communication and writing skills.
  • Ability to work independently and meet deadlines.

Working Conditions and Physical Requirements:
  • Prolonged periods of sitting while working at computer.
  • Frequent walking and standing during presentations.
  • May be required to work outside normal working hours.
