The Patient Representative provides supportive calls to the patients and families at end of life, maintains patient experience data and communicates patient/family needs and concerns to the teams and Quality Review Coordinator. Manages coordination of calls as initiated by workflow and as initiated by primary members of the IDT, noting need to follow up with phone calls and visits. Participates and contributes to the Interdisciplinary Team Process. Displays professionalism during all communication with patients, families and co-workers. Reports all Patient Experience Program shift occurrences and complaints. Qualifications: High School Diploma or equivalent required Bilingual in Chinese (Cantonese or Mandarin) Prior experience working within a healthcare environment, hospice preferred knowledge of hospice HIS requirements Excellent verbal and written communication skills Use of various computer programs including Microsoft Word, Excel, and Power Point
