The Patient Safety Coordinator (PSC) is responsible for the highly advanced (senior level) work of planning, developing, coordinating, and implementing the Texas State Hospitals (SH) critical Patient Safety Oversight program. The PSC reports directly to the Texas State Hospitals Director of Quality and Incident Management. The PSC, with guidance from the QIM Director and the Patient Safety Evaluation System Subcommittee, oversees programs and processes that assess and ensure patient safety for the SHs. The PSC facilitates contracted patient safety trainings between the SHs and the Patient Safety Organization. The PSC conducts trainings on quality improvement methodology with hospitals as needed. The PSC works in collaboration with the Incident Management Coordinator (IMC). The PSC and IMC will reconcile the reporting of incidents in the Texas Safety Net System and email notifications. The PSC works closely with data analytics to provide quality patient safety and incident reports. The PSC and IMC will cross train to provide coverage for each other. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgment. Performs other duties as assigned. Other duties as assigned include actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Essential Job Functions (EJFs): Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Serves as vice-chair of the Patient Safety Evaluation System Subcommittee (PSESS) and Patient Safety Workgroup for Performance Improvement (PSWPI Oversees and updates Patient Safety SharePoint sites with current reports and relevant training materials for state hospitals. Acts as liaison to Texas A & M, Rural and Community Health Institute (RCHI) Patient Safety Organization (PSO #79) for SHS PSO ongoing membership and oversees activities to include contract management, data submission, and report generation. Oversees, updates and maintains the PSES and Incident Reporting policies and procedures in conformity with federal law and PSO requirements Arranges training for and provides consultation to the PSESS and state hospitals on patient safety and federal and state laws related to formal Patient Safety Evaluation Systems and membership in Patient Safety Organizations. Generates reports for Governing Body using patient safety databases (e.g., sentinel events, patient deaths). Provides coverage for the Incident Management Coordinator by reviewing, triaging, escalating pertinent events to Central Office Leadership and responding to leadership queries in a timely manner. In collaboration with the Incident Management Coordinator, assists with management of Abuse, Neglect and Exploitation allegation databases. Performs other duties as assigned. Knowledge, Skills and Abilities (KSAs): Knowledge of federal and state laws and regulations related to hospital patient safety, including the Joint Commission standards and CMS conditions of participation. Knowledge of laws and rules applicable to confidentiality and management of protected health information. Ability to effectively report, investigate, and evaluate patient safety incidents, medical errors, and potential risk using multiple sources of information. Advanced knowledge of medical/clinical/behavioral health processes in an inpatient setting. Skills in interpersonal relationships and promoting a culture of safety. Skills in communicating effectively both orally and in writing. Ability to exercise judgement and work independently with minimum supervision. Ability to lead and work cooperatively in a team environment. Knowledge of SharePoint preferred. Registrations, Licensure Requirements or Certifications: None Initial Screening Criteria: At least five (5) years of experience relating to patient safety, quality, or risk management. Graduation from a four-year college or university with major coursework in a field relevant to the assignment is generally preferred. Additional Information: Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE).
