Be continuously up-to-date and knowledgeable about available community services and establish and maintain relationships with community based organizations. Conduct motivational interviews to identify social determinants and needs. Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement. Using Patient Activation survey/tools, help patients set personal goals and a plan based on their level of activation. Educate and link patients to insurance options, primary care providers, and navigate them to community based services and other resources. Provide patients and families education and literature on wellness, adopting healthy behaviors and available community services. Assist patients with completing survey forms, applications and registration forms.
