Patient Navigator - ATLANTICARE REGIONAL MEDICAL CENTER : Job Details

The Medical Case Management Grant Counselor, provides support, counseling and referral information for patients and families, and performs complete psychosocial assessments as well as mental health and substance abuse screenings. The Counselor demonstrates understanding of community resources and communicates and collaborates effectively with agencies and hospital personnel. The Counselor is also responsible for writing medical record notes in a timely fashion, and building constructive relationships with community service professionals. The Counselor participates in the HIV Planning Group and Ryan White funded quality improvement committees. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.
