Patient Mobility Specialist-5A - UnityPoint Health : Job Details

Contributes toward accomplishment of patient's Plan of Care goals by providing mobility activities as assigned by the nurse. Effectively works with the patient and their support team, reinforcing therapy and therapy/mobility initiatives. Effectively communicates changes in patient condition and functional ability to support an expedited change in the patient's plan of care and prevention of more serious complications. Promotes professional rehabilitation/mobility objectives among interdisciplinary team members. Participates in process improvement for enhancement of care delivery and clinical excellence. Promptly completes documentation to assure an accurate legal record of patient's care. Assist with cleaning/inventorying mobility equipment, retrieving and recommend ordering supplies.
