We are seeking an experienced and professional Patient Care Navigator to become our team. You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Serve patients and caregivers to provide resources and assistance clinical and supportive care services.

What you will do in this role:

• Serve as a single point of contact for referring physicians, patients, and caregivers to ensure access to clinical and support care services.
• Assist patients in understanding their diagnosis, treatment options, and the resources available.
• Assist patients and families in establishing a support network and addressing physical, social, and emotional needs.
• Communicates effectively and works with physicians and/or other members of the interdisciplinary team in meeting patient needs.

What qualifications you will need:

• Bachelors degree in nursing or related health care field.
• Minimum 1 year clinical specialty area experience required.
• Current Florida State Registered Nurse license or appropriate compact licensure.
• Current BLS.
• Critical thinking, service excellence, and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, proficient in PowerPoint and Excel.
