Job Type Full-timeDescriptionUnder general supervision, the Patient Care Coordinator works closely with the Population Health Coordinator and clinic providers in patient outreach and engagement programs to improve quality of care, wellness, and preventive services. Provides program development, patient enrollment, education and outreach. The person in this position is required to work on site. RequirementsQUALIFICATIONS•High school diploma or equivalent•Previous experience in customer service•EMR knowledge preferred•Three (3) or more years in a medical or clinic settingROLE AND RESPONSIBILITIES•Schedule, plan and coordinate patients' annual well visit paperwork with patients.•Coordinate patients' annual wellness visits with nursing staff.•Track and update charts with key quality measures.•Help track and address quality measures with clinical staff.•Update and maintain medication lists in Cerner for nursing home patients.•Treats patients and their families with respect and dignity.•Maintains a good working relationship with all staff members of the clinic and nursing home staff.REQUIREMENTS •Support CMH's mission, vision, core values and customer service philosophy•Adhere to the CMH Compliance Program•Assures/maintains compliance with all accreditations, statutory and regulatory standards at the local, state, and federal levels •Comply with all organizational policies •Demonstrate outstanding customer service/advocacy, professionalism, and integrity•Perform other duties as assigned KNOWLEDGE, SKILLS, AND ABILITIES•Ability to communicate clearly and effectively, orally and in writing. •Strong reasoning/critical-thinking and decision-making skills.•Ability to exercise good judgement.•Ability to complete tasks with critical deadlines.•Ability to establish and maintain effective working relationships with coworkers and diverse patient populations.•Ability to respond in a flexible manner to changing workload or patient assignments.•Proficient Microsoft Office computer skills.•Knowledge of Cerner Revenue Cycle and Cerner Powerchart.•Basic math skills.PHYSICAL REQUIREMENTS AND ENVIRONMENTAL CONDITIONS•Stressful environment•Exposure to odors, blood, body fluids, and excrements, adverse environmental conditions, and hazardous materials•Pushing and pulling heavy objects•Full range of body motion including handling and lifting patients•Position requires light lifting up to 5 lbs.•Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing•Exposure to temperature and humidity changes
