Key Responsibilities:

• Coordination of Care:
• Collaborate with interdisciplinary teams to ensure seamless patient care before and after surgery.
• Monitor clinical processes and interventions to achieve optimal patient outcomes while adhering to financial guidelines.
• Assist in discharge planning from admission, ensuring resources are appropriately coordinated.
• Lead multidisciplinary care planning with a focus on individual patient needs, providing education on community resources.
• Facilitate care conferences for patients with complex health concerns and promote family education throughout the discharge process.
• Quality Improvement:
• Work as part of the team to implement strategies aimed at reducing length of stay and optimizing patient health.
• Identify educational needs within the team and provide relevant training opportunities.
• Engage in critical thinking to develop innovative patient care programs.
• Collaborate with quality improvement efforts to enhance patient outcomes while managing costs.
• Ensure effective communication with community agencies regarding patient care.
• Measurement and Reporting:
• Work with leadership to compile and analyze data on patient care services.
• Communicate findings with the healthcare team and develop strategies for improvement.
