We are Hiring: Patient Care Coordination Associate

We are looking for a friendly, detail-oriented Patient Care Coordination Associate to become an essential member of our Long Island Pediatric Ophthalmology and Strabismus team. In this role, you will assist in following set processes and protocols while delivering excellent customer service. You will work collaboratively with our care team to achieve financial, clinical, and service goals.

Work Schedule: Monday through Friday, with varying hours between 8:00 am and 8:00 pm (one night per week until 8:00 pm), and two Saturdays per month from 8:00 am to 4:00 pm. This is a 37.5-hour work week, determined by your supervisor.

Primary Responsibilities:

  • Gather and verify accurate patient information, including personal and insurance details.
  • Conduct insurance verification on the date of service to ensure seamless operations.
  • Secure necessary patient signatures for documentation.
  • Maintain and organize medical records efficiently.
  • Schedule and confirm patient appointments.
  • Uphold cash security policies in accordance with Optum guidelines.
  • Facilitate timely referral documentation.
  • Assist patients promptly with follow-up services, including scheduling of appointments.
  • Collaborate effectively with team members to achieve shared goals.
  • Adapt to a fast-paced environment while maintaining productivity.
  • Ensure continuity of care through effective scheduling and tracking systems.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Adhere to policies promoting quality care and confidentiality standards including HIPAA and OSHA regulations.
  • Perform additional duties as assigned.

Required Qualifications:

  • High School Diploma/GED or higher.
  • At least 1 year of customer service or healthcare experience.
  • Intermediate proficiency in computer applications, including Microsoft Outlook, Excel, and Word.

Preferred Qualifications:

  • 1+ years in a medical front office or urgent care role handling scheduling, insurance verification, and record maintenance.
  • Experience with electronic health records (specifically EPIC).
  • Familiarity with medical terminology.

Essential Soft Skills:

  • Ability to work independently while exercising judgment and accountability.
  • Multi-tasking prowess and task prioritization skills.
  • Ability to handle high-pressure situations effectively.
  • Strong teamwork and communication skills, both verbal and written.
