Qualifications:
The Patient Care Coordinator is responsible for the coordination of administrative and discharge planning for all patients. Essential duties include formulating continuing care plans with patients, facilitating psycho-education, assisting with paperwork, initiating and completing the referral and intake processes, ensuring completion of exit surveys, maintaining confidentiality, and providing professional, courteous, and helpful responses to calls and mail. The position requires demonstrated poise, professionalism, tact, and diplomacy, and excellent organizational skills with the ability to prioritize workload and meet deadlines. Strong knowledge of computer software applications in word processing, spreadsheets, and presentations is necessary. Understanding of diverse cultures and gender specific issues is also required.
Education and experience: A bachelor's degree in psychology, social work, or related field is preferred, along with a minimum of two years' experience in a mental health/healthcare field. Insurance verification and/or insurance billing experience are a plus.
