Your Role as a Patient Assessment Quality Coordinator
- Ensure accurate data extraction from clinical documentation.
- Coordinate timely submission of data for Medicare patients.
- Educate and support staff on proper and accurate documentation practices.
- Act as the primary resource for problem-solving related to Quality Indicator coding and IRF-PAI completion.
- Work to enhance processes for QI coding and related data collection.
- Guarantee that IRF-PAI data is entered and transmitted accurately.
- Ensure IRF-PAI data is sent to CMS/UDS within required time frames for admission/discharge.
- Review and interpret patient data for IRF-PAI completion.
- Collaborate with the team to identify potential comorbidities or accurate CMG/RIC categories.
- Verify all discharged patient records contain necessary elements.
- Conduct chart audits for timely UDS export and CMS transmission.
Qualifications for Success
- Current license or certification in healthcare (RN, LPN, PT, PTA, OT, COTA, SLP, Recreational Therapist, case manager/social worker).
- QI Credentialed.
- Willingness to obtain UDS IRF PPS Certification within 2 years in the role.
- Minimum of 1 year of healthcare experience.
- Ability to perform assessments effectively.
- Preferred experience in rehabilitation or post-acute care with strong teaching and communication skills.
- Effective oral and written communication abilities.
- Strong organizational and critical thinking skills with attention to detail.
- Commitment to maintaining high standards in patient assessments.
