Apply Today!Salary: Starting at $23.00-$31.00 hourly based on experience.Statement of Purpose: The Patient Access Specialist plays a key role in ensuring patients living with HIV and related conditions can access affordable, timely care. This position is responsible for financial assistance and resource coordination, and enrollment in pharmacy programs, including 340B. The specialist serves as a vital link between patients, providers, pharmacies, and external partners to reduce barriers to care, promote medication adherence, and support program compliance. Uphold CAN's mission, vision, and values so that patients and colleagues experience a consistent, compassionate, and value-based care environment.2. Comply with and enforce CAN policies and procedures so that organizational and regulatory standards are maintained.3. Conduct one-on-one Social Determinants of Health assessments so that individual patient risk factors can be documented and addressed appropriately.4. Educate patients about HIV services and CAN-owned pharmacies so that they can make informed decisions and access comprehensive care.5. Maintain accurate documentation of patient services and follow-ups so that continuity of care and compliance requirements are met.6. Identify and address patient barriers (transportation, housing, etc.) so that patients can access care without interruption.7. Assist with eligibility and enrollment in CAN Cares, Ryan White/ADAP, and public insurance programs so that patients receive the financial support they need for treatment.8. Promote enrollment in CAN-owned pharmacies so that patients benefit from integrated pharmacy support and the organization maximizes 340B program participation.9. Educate clinic about 340B benefits and services so that staff can support patient access to affordable medication.10. Facilitate cost-of-care conversations and serve as liaison between patients, financial counselors, and revenue cycle staff so that patients understand their financial responsibilities and options.11. Help resolve insurance issues and denials related to HIV care so that treatment is not delayed or discontinued.12. Coordinate access to medications for uninsured patients via assistance programs so that no patient goes without necessary medications.13. Complete and submit prior authorizations with accuracy and timeliness so that pharmacy services and treatment are not disrupted.14. Identify and engage patients at risk of falling out of care so that retention and treatment adherence are improved.15. Track and analyze pharmacy reports so that strategies can be implemented to increase utilization of CAN-owned pharmacies.16. Participate actively in all required meetings and training courses so that performance aligns with organizational goals and current practices. 17. Coordinate with pharmacies and providers to manage authorizations, refills, and renewals so that HIV-related Medicare consistently accessible to patients.18. Participates in the quality improvement process (RW CQM).19. Utilizes resources in a cost-effective manner.20. Maintains confidentiality regarding patient/client and/or family standards in accordance with professional and departmental standards.21. Continues professional training and education to advance skills/knowledge of HIV and related disease processes, medications, and treatments.22. Represents CAN Community Health at local, state, and national conferences and meetings, as deemed necessaryWork Environment:This job operates in a professional office environment. This role routinely involves using standard office equipment such as computers, audio visual, telephones, photocopiers, filing cabinets and fax machines.Physical Requirements:1. Neat, professional appearance.2. Exerts up to 50 lbs. of force occasionally and/or 20 lbs. of force frequently and/or 10 lbs. of force constantly in lift, carry, push, pull, or otherwise move objects. Involving sitting most of the time but may involve walking or standing for brief periods of time.3. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.Position Type/Expected Hours of Work:Occasional evening and weekends as needed in service.Travel: Travel is primarily during the business day, although some out-of-area and overnight travel may be expected. Must be able to operate a motor vehicle and have valid insurance and driver's license.RequirementsEducation/Professional:1. Bachelor's degree in public health, Healthcare Administration, Social Work, Psychology, Biology, Global Health, or related field and/or three (3) year minimum of relevant work experience2. Minimum 1-2 years relevant experience (Ryan White, Case Management, Prevention, Pharmacy Technician, etc.)3. Familiarity with HIV services, Ryan White Program, ADAP, and/or 340B pharmacy preferred.Competencies: 1. Communication2. Problem Solving3.
