Position Summary:

As a Social Worker, you will play a crucial role in facilitating healthcare delivery through effective resource coordination. Your aim will be to assist patients in achieving optimal health and access to care while respecting their right to self-determination. You will be responsible for assessing patients' transition needs, identifying those at risk for readmission, and conducting comprehensive psycho-social assessments. This role emphasizes timely throughput, safe discharge, and prevention of avoidable readmissions while ensuring compliance with regulations and accreditation standards.

Key Responsibilities:

  • Conduct complex psycho-social transition planning assessments and interventions
  • Assist with cases involving adoption, abuse, and neglect, including appropriate referrals to agencies
  • Coordinate patient care and implement transition plans
  • Lead multidisciplinary patient care conferences and complex case reviews
  • Make appropriate referrals to various departments and collaborate effectively with physicians and staff
  • Communicate plans of care to patients and families
  • Complete necessary patient education to support post-acute needs
  • Document care processes concisely in the Case Management system
  • Maintain accurate patient demographics and insurance information
  • Mentor new staff and serve as a resource for the team
  • Participate in quality improvement initiatives
  • Other duties as assigned

Qualifications:

Education: Required: Master's degree in Social Work

Experience: Preferred: 2 years of experience in an acute hospital setting

Licensure: Must currently hold or be eligible for LSW or LCSW licensure in accordance with state regulations.
