Key Responsibilities:

• Collaborate with an interdisciplinary team to design personalized treatment plans based on patient needs.
• Coordinate tentative discharge plans and ensure seamless execution for patient discharges.
• Monitor and enhance the patient experience regarding quality of care and service delivery.
• Compile essential documentation and case management reports with accuracy.
• Educate patients and families on rehabilitation processes and establish tailored communication plans.
• Make timely referrals to post-discharge providers and ensure a smooth transition.
• Participate in case management activities, including data collection and trend analysis.

Qualifications:

• Must be licensed or certified to independently conduct assessments within your discipline.
• A degree in Nursing (BSN preferred) or a bachelor's degree in other relevant health care fields is required.
• Two years of rehabilitation experience is preferred.
