Become the Case Manager You Always Wanted to Be
- Collaborate with an interdisciplinary team to tailor treatment plans according to patient needs and preferences.
- Establish tentative discharge plans and coordinate contingency plans with the team.
- Engage in the planning and execution of patient discharge experiences.
- Monitor patient experiences with a focus on quality, timeliness, and appropriateness of services.
- Facilitate weekly team conferences and manage modifications to treatment plans.
- Complete all required documentation meticulously.
- Stay informed of relevant regulations, policies, and operational standards.
- Analyze case management reports to identify trends and implement action plans.
- Comprehend commercial contract levels and payor requirements.
- Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACT occurrences.
- Engage with patients and their families within 24 hours of admission to offer support and information.
- Assess patient goals and document findings within 48 hours of admission.
- Educate patients and families about rehabilitation processes and your role as their Case Manager.
- Schedule and facilitate family meetings as needed.
- Assist patients in obtaining necessary resources to ensure timely discharges.
- Ensure compliance with regulations related to orthotics and prosthetics.
- Make timely referrals to post-discharge providers and document accordingly.
- Ensure accuracy in patient discharge and payor information records.
- Participate in the utilization review process, including data collection and trend review.
- Be available for on-call case management duties as required.
Qualifications
License or Certification:
- Must be qualified to independently conduct assessments within your professional scope (e.g., RN, SW, OT, PT, ST, or Rehabilitation Counseling).
- If relevant, hold an active state license in your discipline.
- Must meet eligibility for CCM® or ACM certification upon entering this position or within two years.
- Possession of CCM® or ACM certification is required within two years of placement in the Case Manager II role.
Minimum Qualifications:
- For nursing roles, an Associate Degree in Nursing is required; RN licensure with a BSN preferred. A diploma is acceptable only in states where a diploma is the minimum licensure or certification requirement.
- For other licensed or certified health care professionals, a minimum of a bachelor's degree is needed; a graduate degree is preferred.
- Preferably, at least two years of rehabilitation experience.
