Become the Case Manager You Aspire to Be

  • Collaborate with an interdisciplinary team to tailor treatment plans based on each patient's unique needs.
  • Establish and adjust discharge plans in coordination with the interdisciplinary team.
  • Engage in weekly team meetings to discuss treatment strategy and necessary modifications.
  • Complete all essential case management documentation in a timely manner.
  • Ensure familiarity with company policies, regulatory standards, and operational procedures.
  • Monitor and analyze case management reports and implement improvements.
  • Educate patients and families about rehabilitation processes and your critical role as their Case Manager.
  • Coordinate thorough pre-discharge planning and assist the patient in overcoming potential discharge delays.
  • Maintain compliance with regulations regarding orders and payments for orthotics and prosthetics.

Qualifications Needed

To excel in this position, candidates should have:

  • An appropriate level of clinical experience, which includes a valid and active license where required.
  • CCM® or ACM certification eligibility upon entering the role, with certification required within two years of placement.
  • A minimum of an Associate Degree in Nursing (BSN preferred) for nursing candidates or a bachelor's degree for other licensed or certified healthcare professionals.
  • At least 2 years of rehabilitation experience is preferred.
