Job Description :

To oversee the Paraprofessional Placement Unit and to ensure the initial placement of all needed paraprofessional services from a vendor contracted by St. Mary's Home Care, and the reassignment of cases to other paraprofessional vendors, whenever warranted. Will develop a good rapport with all vendors to ensure all cases are covered in an effective and timely manner, and are proportionally distributed among vendors.

JOB DUTIES / RESPONSIBILITIES

ESSENTIAL DUTIES

* Manages the Paraprofessional Placement Unit effectively to ensure placement of all approved paraprofessional services.

* Communicates to paraprofessional vendors, new and existing cases that require paraprofessional services, and works to have cases covered in a timely manner. Informs vendors of patients' needs per the Plan of Care.

* 3. Collaborates with vendors and assigned Home Care Team to ensure the level of services, timeframes, hours and days of service are accurately set for cases, and distributes communications, including but not limited to incidents and accidents to vendors and Home Care teams with the agreed upon information.

* Ensures profiles are received from vendor agencies and ensures that profiles are in accordance with policies and procedures. Approves and signs all vendor profiles. Assists with maintaining updated vendor profiles.

* 5. Ensures that accurate orders, schedules, and Aide Plan of Cares are entered into the EMR and is available in selected vendors platforms.

* 6. Monitors changes in paraprofessional services, and acts as centralized resource for clarification of the paraprofessional schedule for each patient for billing purposes. Facilitates resolution of billing issues with paraprofessional services. Follows-up with appropriate department if change is noted.

* Acts as back-up support to Home Care Teams and scheduler for changes in paraprofessional scheduling when Home Care Team assigned personnel or scheduler is out of the office for extended periods of time.

* Notifies and updates Senior management and management staff on the status of cases that have gone without coverage.

* 9. Ensures that skilled professionals are assigned and reminded to complete Home Health Aide Supervisions in a timely manner.

* 10. Assists in assuring all plan of cares are updated and accurate in all on call binders.

* Performs and completes monthly audits to ensure regulatory compliance.

* Assists with closing charts when patients are discharged.

* Ensuring authorization is available for ongoing visits.

* Performs other duties as assigned to support the efficiency and quality of patient care services.

* Supports quality improvement initiatives through data tracking and compliance support.

* Pulls and runs reports as necessary.

* Processes referral information and ensures proper placement of professional and paraprofessional staff.

QUALIFICATIONS / KNOWLEDGE

Formal education required:

* Associates Degree preferably in Computer Science or the equivalent and/or related experience preferred.

* Requires sufficient knowledge of hardware, software, and telecommunications operations, business requirements and area workflow process.

* Strong knowledge in Microsoft Office applications and Cerner.

* Excellent consultative and communication skills, analytical ability, judgment, and the ability to work efficiently with management and staff.
