Position Summary: Under the guidance of the PACE Pharmacists and PACE Clinical Pharmacy Manager, the PACE Pharmacy Clerk provides assistance in maintaining accurate patient medication lists and performs duties related to managed care. Essential Functions of the Job:
- Inputs patient and prescription data in the EPIC electronic medical record system (EMR) to maintain profiles and send to pharmacist for review.
- Participates in daily Interdisciplinary Team Meetings (IDT) for all PACE sites as the Pharmacy representative.
- Notify contracted pharmacies of patient address changes or vacations.
- Supports utilization management by tracking Skilled Nursing Facilities (SNF) discharges, new hospitalizations, and hospice admissions. Updates EMR, Pharmacy Benefit Manager (PBM), and contracted pharmacies of these changes in a timely manner.
- Obtain discharge medication list for patients discharging from hospital and/or SNF and new hospice admissions for pharmacist to perform medication reconciliation.
- Review and upload Stay A data to the PBM's secure website weekly.
- Gather and submit monthly enrollment data to contracted pharmacies and PBM. Monitor any enrollment changes daily to update contracted pharmacies and PBM.
- Assists Pharmacists to ensure new patients have complete medication list documented on EPIC EMR by checking prior clinical notes or calling patients/caregivers.
- Assist pharmacist in identifying immunization needs and send to pharmacist for review.
- Assists pharmacist by calling specialist providers offices for visit notes or clarification questions and report findings to pharmacist.
- Support clinic staff with medication concerns and following up on clinic use supply orders with pharmacy.
- Assist with training new pharmacy staff on multiple EMR systems.
- Assists and services patients, including picking up/distributing medications for patients.
- Communicates with patients/caregivers regarding prescription needs, order and delivery status.
- Assists in the preparation of reports and written documentation, as required by the department, company, state and federal agencies.
- Other duties as assigned.

- Enhances professional growth and development through participation in educational program, current literature, in service meetings and workshops.
- Attends meetings as required and participates in committees as requested.
- Performs other related duties as assigned or requested.
Job RequirementsEducation Required:
- High School or GED equivalent.
Experience Required:
- 1 year experience in an outpatient pharmacy, hospital pharmacy or retail setting.
- Preferred: 1 year in a PACE or managed care setting.
Certifications Required:
Verbal and Written Skills Required to Perform the Job:
- Excellent oral, written communication, and interpersonal skills.
- Bilingual (English/Spanish) preferred.
Technical Knowledge and Skills Required to Perform the Job:
- Familiar with medical/pharmaceutical terminology.
- EPIC EMR experience/knowledge preferred.
Equipment Used:
- General office equipment to include; Personal Computer, Software Systems, Scanner, Telephones, Copy and Fax machines.
Working Conditions and Physical Requirements:
- Prolonged periods of sitting and constant walking and standing.
- Driving and occasional travel required.
- Superb organization ability and detail oriented.
