Description
The Enrollment Specialist holds primary responsibility for orienting potential participants and community partners about NEMS Program of All-Inclusive Care for the Elderly (PACE) and intake of new enrollments. The role involves gathering information, setting appointments for assessments, and documenting needs for incoming cases. The specialist interacts with referral sources, community organizations, family caregivers, and participants, providing presentations, tours of the PACE center, and collecting information related to medical and social needs as the first step in enrollment.

Hourly Rate: $32.08 - $36.15
Essential Job Functions
- Serve as the first point of contact between the PACE team and potential participants and referral sources.
- Provide information about PACE and answer questions to educate about services.
- Gather necessary information and determine enrollment eligibility through screening and intake.
- Identify and document medical and social indications for new participants.
- Assist with the enrollment process and level of care assessments.
- Educate referral sources and potential participants about NEMS PACE benefits.
- Conduct tours of the PACE Center for potential participants and families.
- Present cases to the Interdisciplinary Team.
- Develop relationships with referral sources and support enrollment goals.
- Interact professionally with older adults, including those with cognitive or physical disabilities.
- Identify marketing and outreach opportunities.
- Collect demographic and required paperwork within designated timeframes.
- Utilize Epic efficiently and serve as a workflow expert.
- Perform other duties as assigned.
Qualifications
Required:
- At least one year of experience in sales or community outreach.
- Current BLS and First Aid certification.
Preferred:
- Bachelors degree in Healthcare, Social Sciences, Business, or related field.
- Experience with frail or older adult populations.
- Experience giving community presentations.
- Experience working with older adults in healthcare or community settings.
Language Skills
- Fluent in English (reading, writing, speaking).
- Fluent in Chinese (Cantonese and/or Mandarin) is required.
- Other languages are an asset.
Status
- FLSA non-exempt, full-time position.
- Not an OSHA high-risk position.
