Position Summary: Under the direct supervision of the PACE Assistant Center Manager, the Companion Coordinator is responsible for managing all companion requests, scheduling, and execution of companionship services during specialty appointments. The role ensures optimal efficiency in providing these services. Essential Functions of the Job:
- Oversee daily operations related to companionship by assigning tasks to staff, ensuring efficient service delivery and a positive experience for participants.
- Maintain a daily log of staff assignments and allocate resources to ensure comprehensive provision of companionship services.
- Prepare and provide concise reports, keeping center leadership informed of key metrics.
- Engage in the onboarding, training, and mentoring of PCAs and CNAs on SD PACE/SYH's mission, vision, values, Day Center objectives, and workflows.
- Collaborate with the Assistant Center Manager to communicate job expectations, manage staff performance through coaching, counseling, and disciplinary actions, conduct performance evaluations, prepare work schedules, and provide necessary resources.
- Contribute to the development and implementation of departmental policies and procedures to meet participant and care team objectives, promoting a quality-focused environment that achieves performance and care goals.
Additional Duties and Responsibilities:
- Maintains and complies with established SYHealth and departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
- Performs other related duties as assigned or requested.
- Attends meetings as required.
Job Requirements:Education Required (Minimum level of education):
- Required: High School Diploma or GED equivalent.
- Preferred: Post-high school vocational or specialized training.
Experience Required (Minimum level of experience):
- Minimum of 1 year working with seniors in an adult day health center setting or related care setting. Relevant education or volunteer work may substitute for experience.
Experience Preferred:
- Clinical experience
- Experience with a PACE Program or adult day health center setting
- One year caring for seniors or elderly in an assisted living program, nursing home that provides services to disabled or elderly adults.
Verbal and Written Skills Required to Perform the Job:
- Strong verbal and written communication skills. Bilingual in Arabic or Spanish preferred.
Technical Knowledge and Skills Required to Perform the Job: Must possess the skills necessary to provide care for geriatric patients, must thrive in Team environment, must possess good organizational and supervisory skills, ability to effectively handle difficult and unusual interpersonal situations. Meet a standardized set of competencies for the specific position description established by SYHC PACE and approved by CMS. Equipment Used: Personal computer, Electronic Health Record system, or computerized system. Telephone devices and voicemail. At a minimum level have good computer literacy including internet navigation and research skills. Working Conditions and Physical Requirements:
- Must be comfortable with constant walking and standing
- Ability to work in a noisy environment and lift up to 25 lbs.
- Willingness to work evenings, weekends and holidays as needed
