Utilizing both Trauma-informed care and peer support techniques, the Outreach/Peer Support Intake Specialist will connect with individuals in the community and work to connect them with ACCESS shelter and community navigation programs. If you are dedicated, hardworking and have a passion for helping others, ACCESS is the place for you!Position Duties & ResponsibilitiesInterview, screen, and work with potential participants seeking support in the ACCESS Outreach programs.Perform administrative duties to support the program.Input accurate information promptly into multiple databases as necessary.Assemble and maintain files and reference materials as instructed by ACCESS procedures.Perform documentation and file reviews for data Update program policies and procedures as instructed by Program Leadership.Process and review program applications according to ACCESS program procedures, accurately and promptly.File, maintain, and/or shred documents according to ACCESS procedures.Coordinate with other Departments and/or Agencies to provide referrals for programs/services in the community to ensure maximum service to participants.Return phone calls from the Outreach phone in accordance with ACCESS procedures and time frames.Complete other duties and special projects as assigned by the Program Leadership.Ensure continuity of care for all participants entering the sheltering program.Perform a leadership role in the Medford Severe Weather shelter during inclement weather activations.Work cohesively with the shelter Peer Support team and Shelter program facilitator on the program intake process.Position RequirementsBelief in ACCESS' mission to provide food, warmth, and shelter.Must be able to multitask in a variety of areas and maintain sustained concentration with attention to detail.At least 1 year of experience working with vulnerable populations and evaluating eligibility or a similarly complex environment.Must have good interpersonal, written communication, and oral communication shills.Demonstrated ability to work effectively with customers and co-workers and sustain participant and interdepartmental confidentiality.Able to work both independently and collaboratively.Demonstrative excellent record-keeping and organizational skills with a minimum of six months' experience.Excellent computer skills including proficiency in software programs including but not limited to Microsoft Office products and web-based applications.A Valid Oregon driver's license with a record that is acceptable under ACCESS' insurance policies.Equal Employment Opportunity (EEO) Statement“ACCESS is an Equal Opportunity Employer.
