Job Description Summary:
The People Concern's Malibu Outreach Team is seeking a Case Manager to join our multidisciplinary outreach team. This team is part of a City of Malibu strategy to combat homelessness. The goal of the Malibu Outreach Team is to identify, engage, and connect or re-connect homeless individuals to interim and/or permanent housing and supportive services. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment. The Malibu Outreach Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in Malibu. The Outreach Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Outreach Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.
