We provide a range of mental health and substance abuse direct services and case management. Description GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:Assesses psychological and treatment needs of assigned clients. Provides intensive therapeutic services to individuals in assigned settings. Collaborates with other members of the treatment team, design and directs the implementation of the treatment plan which is individualized to each client and their individual needs.ESSENTIAL & CORE FUNCTIONS:1. Develops comprehensive treatment plans that are goal directed, measurable, and achievable.2. Provides individual/family and/or group therapy sessions to assigned consumers based on weekly productivity requirements.3. Evaluates individual consumer needs, interests and response to treatment.4. When applicable, recommends and makes appropriate referrals to other programs within and outside the Agency; completes all program documentation forms, including incident reports.5. Attends all scheduled supervision sessions with immediate supervisor or designee.6. Coordinates and attends case conferences.7. Attend meetings and seminars as directed by supervisor.8. Conduct periodic reviews of client plans as needed. Ensure appropriate levels of medically necessary services are provided. Complete initial assessments for clients CBE as required.9. Actively participates in staff meetings. In accordance with Community Council and licensure requirements, attends in-service training to achieve, maintain and enhance professional competence. Interacts professionally with the clients, co-workers, families and/or community members.10. Adheres to policies and procedures Community Council.11. Performs all duties independently. May be required to report to alternate sites as assigned.12. Performs related duties, special assignments and projects as required by Community Council management.13. Ability to work with all levels of directors, supervisors, colleagues, staff, and consumers in an effective and positive manner.14. Ensures excellent customer service delivery within the Program/Department.Position Requirements PREREQUISITES & QUALIFICATIONS FOR THE POSITION:A person who meets one of the following:(i) Has a graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes six credits of a clinical practicum.(ii) Has an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. (AICE) or the National Association of Credential Evaluation Services (NACES).
