Job Description :

We are seeking a compassionate and motivated Oral Health Navigator to support members access the care they need in dental care. In this role, you will help reduce barriers to oral health services, connect members with dental providers and social supports, and improve oral health literacy. This is an excellent opportunity for someone who is passionate about community health, patient advocacy, and making a direct impact on people's lives.

Key Responsibilities

  • Build trusting relationships with members and assess their oral health needs and barriers to care.
  • Help members find participating dental providers and secure appointments, including arranging transportation and translation support.
  • Provide education on dental benefits, oral health practices, and home care instructions.
  • Support members in overcoming barriers such as dental anxiety, health conditions, and social challenges.
  • Collaborate with health systems, state agencies, and community organizations to connect members with additional services.
  • Document care coordination activities in compliance with program standards.
  • Participate in team case conferences and contribute to solutions for improving outcomes.

Qualifications

  • Bachelor's degree in social work, human services, community health, or a related field strongly preferred.
  • Experience in care coordination, case management, or health navigation (especially within Medicaid, behavioral health, or community health settings) preferred.
  • Commitment to culturally sensitive and member-centered care.
  • Strong communication, organizational, and problem-solving skills.
  • Proficiency with Microsoft Office and ability to document care activities accurately.
  • Bilingual skills a plus.

Location: Worcester, MA (Hybrid – 2 days in office required)

Schedule: Full-Time, 37.5 hours/week

Salary: $50,000 - $60,000 DOE
