Located in the heart of Colorado, our skilled nursing facility provides rehabilitation, long-term care, and support in a warm, team-oriented environment. We are looking for a dedicated Occupational Therapist to join our dynamic therapy team. Position Summary The Occupational Therapist (OT) will evaluate, plan, and implement therapeutic programs to help residents achieve their highest level of independence in activities of daily living (ADLs), functional mobility, and overall quality of life. The OT collaborates closely with residents, families, and the interdisciplinary team to ensure individualized, holistic care. Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide occupational therapy interventions to improve functional skills and independence.
- Document progress notes, treatment plans, and discharge summaries in compliance with state and federal regulations.
- Educate residents, families, and caregivers on adaptive techniques and equipment use.
- Collaborate with physicians, nurses, and interdisciplinary staff to support resident goals.
- Participate in care conferences and contribute to quality improvement initiatives.
Qualifications
- Current Colorado Occupational Therapist license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Previous experience in skilled nursing or geriatric care preferred, but new graduates are welcome to apply.
- Strong interpersonal, communication, and organizational skills.
- Commitment to resident-centered care and teamwork.
