Occupational Therapist Key Responsibilities:

• Conduct assessments and develop individualized occupational therapy treatment plans
• Provide occupational therapy services in outpatient centers and in the field (homes, community, nursing facilities)
• Document evaluations, progress notes, goals, and discharge summaries in compliance with program policies
• Collaborate with an interdisciplinary care team to support holistic, person-centered care
• Recommend and train participants in the use of adaptive equipment and assistive devices
• Support discharge planning and ensure continuity of care through home safety evaluations and care coordination
• Participate in quality improvement, staff development, and peer training programs
• Maintain a clean, safe environment and follow all safety protocols and regulatory standards

Occupational Therapist Required Qualifications:

• Graduate of an accredited Occupational Therapy program
• New York State licensure as an Occupational Therapist
• At least 1 year of experience as an Occupational Therapist preferred
