Occupational Therapist - Switch4 LLC : Job Details

Title: Occupational Therapist Location: La Quinta, CA Job Type: FTE/Permanent Job Summary: The Occupational Therapist focuses on coordinating and providing restorative and rehabilitative occupational therapy services within the facility and at home. The Occupational Therapist works alongside the rehabilitation team to maximize participant independence, safety, and restore function. Essential Job Duties:

• Conduct Occupational Therapy assessments to define care needs, preferences, and goals.
• Actively participate in team meetings to develop participant care plans.
• Deliver and document Occupational Therapy interventions in accordance with physician orders and/or as agreed upon in care plans, including but not limited to the use of devices, techniques, and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and other factors affecting the level of independence in performing ADLs.
• Supervise Therapy Assistants, including providing training, instruction, and guidance for implementing Functional Maintenance Programs (FMP).
• Provide training and instructions to staff, participants, and caregivers/families in areas of Occupational Therapy they may work with participants.
• Work with other team members to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home) and participate in end-of-life care coordination and support.

Job Requirements:

• Bachelor's Degree in Occupational Therapy from an accredited program.
• Two (2) years of occupational therapy experience in a clinical setting with a frail or elderly population.
• Two (2) years of supervisory experience with demonstrated ability to mentor and develop team members preferred.
• Registration from the National Board for Certification in Occupational Therapy.
