Every innovation, medical breakthrough, and groundbreaking treatment is powered by some of the best minds in medicine, ensuring nothing is left on the table or the road to recovery.A Brief Overview Evaluates and develops an appropriate Occupational Therapy plan of care for individuals referred by a physician and assigned by the Home Care Agency to restore function and prevent disability following disease, injury or loss of body part. Helps patient reach maximum performance within their disabilities.What you will do
- Evaluates the patient's level of function and occupational therapy needs and creates a Plan of Care including interventions, goals, and outcomes in collaboration with the patient's medical provider.
- Complete a Comprehensive Assessment as required by the Conditions of Participation for Initial or Start of Care visits which includes verification of eligibility for Home Care benefit including homebound status, complete the Medication Reconciliation and Admission packet including signed consents and incorporate OASIS items in that assessment.
- Serve as Case Manager on cases that are deemed Therapy-Only once the skilled need is established by the skilled nurse, physical therapist or speech and language pathologist.
- Directs and aids patients in active and passive therapeutic exercise, muscle re-education and activities of daily living/self-care to promote independence.
- Maintains visit frequency as ordered and notifies medical provider, Case Manager and Clinical Supervisor of any changes in visit patterns or treatment plan.
- Instructs patient/family/caregivers in care and use of Assistive and Adaptive equipment including, but not limited to: Hoyer lift, wheelchair, brace, crutches, cane, prosthetic and orthotic device.
- Incorporates Patient Safety in all assessment, instructions and training and recommends Home Modifications as identified.
- Verbally instruct and provide a customized Home Exercise Program to patient, family/caregiver and/or Home Health aide and provide educational materials as indicated.
- Provide the needed documentation for pre-authorization of services, monitor insurance authorization and request for additional visit authorizations as needed.
- Communicates patient progress, needs and concerns to Nurse Case Manager, medical provider and other team members and documents such communication.
- Participates in staff meetings and other educational venues and in all agency educational functions including orientation of new staff, field visits with students, and Home Health Aide monthly classes.
- Demonstrates a commitment to own work schedule by adhering to attendance standards and obtaining coverage for cases during illness or vacations.
- Provides care and services to reflect a commitment to the HNMC Mission Statement.
- Completes all tasks including Documentation and other pertinent projects in a timely manner in compliance with HNHC policies.
- Coordinate with Assisted Living Facility staff in the provision of Occupational Therapy services.
- Pursues Evidence Based Practice standards and applies to clinical practice.
- Participates in Caseboxing, Interdisciplinary Case Conferences, Risk Management and Performance Improvement activities.
- Performs additional related duties as assigned.
Education Qualifications
- Bachelor's Degree in Occupational Therapy Required
- Master's Degree in Occupational Therapy Preferred
Experience Qualifications
- 1 year of clinical experience Required
Knowledge, Skills, and Abilities
- Ability to treat patients of all ages, types of disabilities, and with multiple medical problems
- Ability to apply appropriate body mechanics and use of assistive equipment in all patient transfers
- Ability to comprehend spoken and written English, to communicate with clarity, work independently with non-combative and consistent behavior, good assessment and documentation skills, good understanding of therapeutic techniques, ability to interact with a mixed patient/staff population.
- Ability to use Computer Software System for All Patient Care Documentation and synchronize the computer at least twice daily.
Licenses and Certifications
- OT - Occupational Therapist – Registered Valid Occupational Therapy license from state of NJ Upon Hire Required
- AOTA - Occupational Therapy Certified or eligible for certification by the American Occupational Therapy Association Upon Hire Required
