Patient Care
Leads nursing practice using the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing to promote the patient experience.Demonstrates proficiency in Purposeful Hourly Rounding (PHR) and role models expected behaviors.Ensures integration of PHR into clinical activities to enhance Relationship Centered Care.Validates staff compliance with hourly rounding by direct observation and patient feedback.Assesses staff competency in PHR and provides consistent, meaningful feedback.Rounds on all patients daily (Monday–Friday).Communicates patient experience and staff engagement data to staff and the Interdisciplinary Patient Care Team; collaborates on action plans.Partners with physician dyad and escalates concerns as appropriate.Follows up on patient concerns and provides service recovery as needed.Demonstrates caring, respect, compassion, empathy, and active listening through communication and actions.Patient Experience
Role models Mount Sinai Relationship Centered Care (MSHRCC) practices, including Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, PHR, Medication Review, Discharge Wrap-Up, and MD/RN collaboration.Conducts staff huddles at the start of shifts to identify high-risk patients.Communicates issues identified during huddles and throughout shifts to nursing leadership.Initiates service recovery for patient/family concerns and escalates to leadership as needed.Interprets and communicates patient experience scores/benchmarks; updates staff on progress and opportunities.Considers patients' values, preferences, cultural diversity, needs, and knowledge in all aspects of care.Empowers staff to use evidence-based practices to improve patient perception of care.Consistently demonstrates compassion, empathy, and respect through verbal and non-verbal actions.Skills & Competencies
Quality and Safety
Designs and leads a safe, therapeutic, and efficient patient-centered care environment.Investigates practice/operations issues through RCA and implements corrective actions.Encourages a fair, open culture for reporting errors and near-misses.Leads interdisciplinary quality improvement teams using high reliability principles.Reviews compliance with quality and safety metrics with clinical nurses and healthcare team members.Responds to safety concerns/escalations per institutional protocols.Monitors IT infrastructure and corrects gaps in clinical nurse documentation.Leads staff participation in quality/safety initiatives and compliance measures.Ensures accountability for all unit quality and safety practices, staff competencies, and compliance.Education & Experience Requirements
Education:
- Bachelor of Science in Nursing (BSN) required
- Master's in Nursing (MSN) or a health-related field required
Experience:
- 3–5 years of nursing clinical care experience
- Charge Nurse/Supervisory experience preferred
- Relevant clinical competence in the assigned nursing practice area
- Prior work with the addiction population required
- Travel nursing experience required
Licensing & Certification:
- Active RN license with current registration in New York State
