Nurse Practitioner (NP) Position

As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You'll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You'll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.

Position Details:

  • Complete Comprehensive Health Assessments
  • Manage Complex Patients, Focused on Reducing Hospitalizations and Improved Health Outcomes
  • Monday-Friday Schedule with the Ability to Accommodate Patient and Dialysis Centers Availability
  • Must be Flexible
  • Full-Time Self-Managed Schedule

Requirements:

  • Passion for Delivering Care to Patients with Complex, Chronic Illnesses
  • Minimum of Two (2) Years' Experience as a Nurse Practitioner (NP)
  • Current Unrestricted NP License to Practice in the State
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) Certification
  • Intermediate Computer Skills and Proficiency in MS Word, Excel & Outlook Required
  • Home Office and High Speed Internet
  • Willingness to Obtain Up to Six Additional State Licensures for Remote Management of Patient Population

Preferred Qualifications:

  • Preferred Experience Working in Value Based Care or Population Health Including Documenting and Coding from a MA (Medicare Advantage) Risk Perspective and Conducting Health Risk Assessments on High Risk/Chronically Ill Patients
  • Five (5)+ Years' Experience as a Nurse Practitioner Practicing in the Adult/Gerontology Populations
  • Spanish-Speaking Preferred in Some Markets
  • Experience with Longitudinal Management of Chronic Illness (CHF, DM, CV Disease, HTN)
