Bronx/ Manhattan Schedule: M-F, 8 am to 5 pm? Temp (undetermined contract length) Flex. Staff is seeking a Nurse Practitioner Supervisor (NPS) for our client, a non-profit healthcare organization providing home and community-based healthcare and services for the elderly. PURPOSE: The NPS collaborates with community primary care physicians (CPCPs) to oversee and coordinate participant care within the Interdisciplinary Team (IDT). This role focuses on participant independence and wellness, acting as a liaison between the participant, CPCP, and plan staff to develop and implement care plans. RESPONSIBILITIES: Supervise and manage Nurse Practitioners and other clinical staff at assigned PACE site. Conduct performance appraisals. Represent/serve as Medical Director in their absence or as directed. Perform annual wellness assessments (more frequently for high-risk participants). Provide urgent care and same-day access.
