Saratoga Hospital is recruiting for a Geriatric opportunity working at The Wesley Community - Health Care Center (Nursing Home) This is a Full-time opportunity, 8 - 4:30 Monday - Friday. Compensation: $116 K - $150 K (Tiered by years of experience.) Saratoga Hospital seeks Geriatric Nurse Practitioners or Physician Assistants to care for patients at Wesley Community - Health Care Center (Nursing Home). Work with an additional provider, and care for 12 to 15 residents daily for their monthly follow-up visit, and see residents for acutely ill residents. Review MOLST, medicine changes, advanced care plan, etc. with resident. Manage chronic conditions of diabestes, heart failure. No admissions are performed by APPs. APPS will provide support for our Geriatric physician whose duties include seeing residents on a regular rotating schedule. All residents in long-term care are seen once per month.
