Position Overview: We are looking for a passionate and skilled Nurse Practitioner (NP) for a per-diem position at our Fountain House location in New York City. Join our dedicated team to provide comprehensive, patient-centered care. This role entails diagnosing, treating, and educating patients with a focus on prevention and wellness.

Key Responsibilities:

  • Patient Care & Assessments: Conduct thorough physical examinations, gather detailed patient histories, and create individualized treatment plans. Order lab tests, perform blood draws, and conduct necessary diagnostic tests. Diagnose and treat minor illnesses and prescribe medications as needed.
  • Patient Education & Counseling: Inform patients about preventive care, lifestyle changes, and disease management. Provide counseling on chronic conditions, medication adherence, and mental health management.
  • Documentation & Compliance: Maintain accurate and timely documentation of patient interactions and treatment plans, adhering to organizational and regulatory standards.
  • Team Collaboration: Engage in interdisciplinary case discussions and contribute to continuous quality improvement initiatives aimed at enhancing patient outcomes.
  • Clinical Oversight & Safety: Follow OSHA infection control standards and ensure a safe healthcare environment for both patients and staff, while meeting productivity and satisfaction standards.
  • Special Patient Needs: Deliver care across a wide range of age groups, ensuring that age-appropriate services are provided to infants, children, adolescents, adults, and geriatrics.

Minimum Qualifications:

  • Education: Master's degree in nursing from an accredited institution.
  • Licensure/Certification: Current Nurse Practitioner license in New York State and CPR/BLS certification.
  • Experience: A minimum of 5 years of clinical experience as a Nurse Practitioner, ideally within an FQHC or large medical practice.
  • Specialized Skills: Proficiency in performing physical exams, managing chronic illnesses, and prescribing medication. Experience in patient education, particularly in chronic disease management is preferred.

Location: Fountain House, New York City, NY Position Type: Per-Diem Salary Range: $74.46 - $78.07 per hour based on qualifications.
