Deliver palliative care services to terminally ill adult patients across inpatient and community settings Manage a patient panel with an emphasis on continuity of care alongside fellow N - Ps Collaborate closely with interdisciplinary teams to ensure compassionate, holistic care Support patients and families through symptom management, advanced care planning, and end-of-life decision-making Qualifications: Current NP license in New York State Certification as a Palliative Care NP strongly preferred Will consider AGN - Ps (preferred) and FN - Ps with relevant experience New graduates welcome to apply if experienced in oncology, hospice, or palliative care as an RN Salary: The posted range is not a guarantee.
