Nurse Practitioner - Charles B. Wang Community Health Center : Job Details

Responsibilities:Involve with direct patient physical assessment, education, and health planningMaintain continuity of care by regular patient follow-upAssist in the coordination of services with other disciplines and patient's insurance plansAdvocate for patient in arrangement of medically necessary servicesCommunicate with community's resources to ascertain adequate assistance for patients in needRefer patients to specialty services when necessaryConsult with collaborating physician and other members of the health care team to establish plan of careParticipate in quality assurance process to assure optimal care deliveryProvide comprehensive case management services that include emphasis on health promotion and disease preventionMonitor patient outcome in accordance to the practice protocolsCollaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planningAct a resource person for the nursing staffConduct education in service on practice issuesPerform other duties as assigned Minimum Qualifications:Graduated from approved Nurse Practitioner ProgramLicensed as Nurse Practitioner in New York StateBasic CPR (BCLS) certification requiredAt least 1 to 2 years of clinical experience as a primary care Nurse PractitionerBoard/Specialty Certification preferredKnowledge of a second language (such as Mandarin, Cantonese, Korean, Spanish) preferredCompensation and Benefits:Salary: $105,000.00 - 115,000.00 per yearProductivity-based incentive payMedical insuranceEmployer-paid dental, disability and life insuranceEmployer-paid malpractice coverage through FTCA403(b) retirement savings planFlexible spending accounts for transit, parking, health and dependent carePaid time off (6 weeks)Continuing medical education time and allowanceEmployee Assistance ProgramNational Health Services Corps loan repayment program available, andThe Health Center is qualified for Public Service Loan Forgiveness (PSLF) program
