Nurse Practitioner– Full Time – Internal Medicine - Broadway Practice - Ambulatory Care Network/Columbia Campus

The Broadway Primary Care Practice is designated by the National Committee for Quality Assurance as a Level 3 Patient-Centered Medical Home. This innovative model of care delivery is designed to enhance the community's access to outpatient services and improve coordination with the Hospital's inpatient programs.

The Broadway Practice has transformed how we provide care to the Washington Heights community through its successful pilot of the Interdisciplinary Care Team, core to the Patient-Centered Medical Home model. All members of the care team—Nurses, Nutritionist, Care Manager, Community Health Worker, Patient Financial Assistants, Social Worker, Medical Assistants, and Physicians collaborate to improve the health and well-being of our patients.

Use your expertise and passion for educating, providing services and care to patients of all ages and empowering them to stay healthy. You will work with a dynamic team to optimize the patient's health and wellness. Here, nursing requires an unwavering commitment to excellence and a constant spirit of professionalism.

This is a full time, Monday through Friday position – 8:30 a.m. to 5:00 p.m. Hours will vary.

Preferred Criteria:• Minimum three years of experience as a Nurse Practitioner in Internal Medicine • Out-patient experience• Bilingual - English/Spanish

Required Criteria:• Master's degree in Advanced Practice• BSN• Current NYS RN • Current NP licensure in area of specialty practice• BLS through AHA• Minimum of two years of ambulatory care NP experience• Strong communication, interpersonal, and triage skills

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

$154,468-$165,793/Annual
