Complete Comprehensive Health Assessments. Manage complex patients, focused on reducing hospitalizations and improved health outcomes. Monday- Friday schedule with the ability to accommodate patient and dialysis centers availability. Must be flexible. Full-Time self-managed schedule. No nights, weekends and on-call!
