Position Overview: Sun River Health is looking for a dedicated Nurse Practitioner (NP) to join our dynamic team at the Richmond Hill Health Center. The NP will collaborate with a physician to deliver comprehensive, patient-centered healthcare, emphasizing preventative care and patient education. This role involves planning, evaluating, and providing healthcare services while upholding the highest standards of care in line with our mission.
Key Responsibilities:
- Patient Care: Conduct thorough patient assessments, including medical histories and physical examinations.
- Prevention & Education: Promote preventative health measures by educating and counseling patients on health maintenance and lifestyle changes.
- Clinical Care: Order and interpret diagnostic tests, prescribe treatments for minor illnesses, and deliver follow-up care. Perform blood draws and laboratory tests as necessary.
- Collaboration: Collaborate closely with physicians and other healthcare providers to deliver high-quality care, making referrals and consulting with team members as needed.
- Documentation: Maintain precise and up-to-date patient records, ensuring documentation meets organizational standards and regulatory requirements.
- Patient Monitoring: Proactively follow up on abnormal results, like pap smears and reportable STDs, ensuring prompt patient care.
- Case Conferences & Quality Improvement: Engage in case conferences, continuing education programs, and quality improvement initiatives.
- Infection Control: Comply with OSHA and CDC guidelines for infection control and safety standards, ensuring a safe environment for staff and patients.
- Age-Specific Care: Provide care tailored to patients' age-specific needs, from infants to geriatrics.
- Teamwork & Leadership: Work with a multidisciplinary team to achieve productivity and quality standards, maintaining high patient satisfaction.
Job Title: Nurse Practitioner Location: Richmond Hill Health Center, Queens, NY Position Type: Full-Time Salary Range: $145,197 - $152,236.50 (depending on experience and licensure)
Minimum Education & Experience Requirements:
- Education: Master's degree in nursing from an accredited program.
- Licensure/Certification: Valid New York State Nurse Practitioner license.
- Experience: At least 5-8 years of clinical experience, preferably in a large medical practice or community health setting.
- Preferred Experience: 8+ years of clinical experience, with a solid background in primary care or family medicine.
Specialized Skills:
- Bilingual (English/Spanish) preferred, with strong oral and written communication skills.
- Proficient knowledge and experience in diagnostic and treatment procedures, and health education for diverse patient populations.
- Able to collaborate effectively with other healthcare providers for continuity and quality of care.
- Experience in delivering comprehensive healthcare to underserved communities.
