SUMMARY Family Service League is seeking a part-time PsychiatricNurse Practitioner for the Assertive Community Treatment (ACT) program. The Psychiatric Nurse Practitioner will assess the mental health status of clients by studying their relevant medical history, performing comprehensive assessments, and identifying risk factors that might affect mental health. This position diagnoses patients' mental illness and determines effective plans of care. The ACT East (Riverhead) program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. The Psychiatric Nurse Practitioner will be given an agency vehicle to use for ongoing home visits with the clients. - The Psychiatric Nurse Practitioner will prescribe and revise psychopharmacology regimens.
- Interpret findings and plans of care according to collaborative practice agreements and protocols in designated areas.
- Administer and monitor medications, including injectable medication.
- Psychiatric Nurse Practitioner will conduct psychiatric evaluations.
- Conduct health assessments of adults with potential, acute, and/or chronic health problems.
- Initiate referrals to specialty services and foster continuity of care.
- The Psychiatric Nurse Practitioner will initiate selected therapeutic and diagnostic procedures, consultation, and laboratory examinations.
- Provide patients and their family members with education and counseling in the areas of health promotion, maintenance, and disease prevention.
- Participate in team meetings and conferences to enhance an interdisciplinary approach to primary health care delivery.
- The Psychiatric Nurse Practitioner will serve as liaison between the patient and the community, and act as a mental health care advocate.
- Identify quality improvement issues and document standards of care and practice.
- Maintain a current level of knowledge relative to professional practice.
- All other duties as assigned.
QUALIFICATIONS Master's Degree from a Psychiatric Nurse Practitioner Program required. Current license to practice as a Registered Professional Nurse and a Psychiatric Nurse Practitioner in New York State required. National Board Psychiatric Certification. DEA credentials required. Candidate must possess a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education. At least 1 year of clinical experience in a behavioral health setting required. A minimum of 3 years of prior prescribing experience is required. Psychiatric prescribing experience is preferred. Proficient computer skills, including Microsoft Office required; experience with an EMR is preferred. Excellent time management, interpersonal, and verbal and written communication skills required. Valid and clean New York State Driver's License required.PHYSICAL REQUIREMENTS This position requires sitting for long periods of time and travelling to conduct home visits.
