Maimonides Medical Center is seeking a dedicated Age-Friendly Program Manager (AFPM) to lead the implementation and sustainability of Age-Friendly initiatives across the organization. This role focuses on advancing the 4Ms framework (Mentation, Medication, Mobility, and What Matters) through governance structure development, interdisciplinary collaboration, performance tracking, and quality improvement initiatives. The AFPM will spearhead efforts to enhance transitions of care, improve post-discharge follow-ups, strengthen Skilled Nursing Facility (SNF) partnerships, and oversee caregiver engagement initiatives as part of the AIM 2.0 program.

As a key liaison, the AFPM will connect internal stakeholders (e.g., Nursing, Geriatrics, Surgery, Emergency Medicine, Palliative Care, Medical Informatics) with external partners (e.g., SNFs, caregivers, community organizations) to ensure seamless continuity of care and promote patient-centered outcomes.

Specifically, you will:

  • Lead the development and sustainability of Age-Friendly Health System initiatives across MMC.
  • Establish governance structures to align Age-Friendly programs with organizational objectives.
  • Foster interdisciplinary collaboration to integrate the 4Ms framework into clinical workflows.
  • Track performance metrics, report outcomes, and drive quality improvement initiatives.
  • Enhance transitions of care, post-discharge follow-ups, and SNF partnerships.
  • Oversee caregiver engagement initiatives within the AIM 2.0 program.
  • Serve as a liaison between internal teams and external partners to support continuity of care.

We require:

  • Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field.
  • Minimum 3+ years of experience in healthcare program management, quality improvement, or geriatric care initiatives.

We prefer:

  • Master's degree in Health Administration, Public Health, Geriatrics, or a related field.
  • Certification in Age-Friendly Care, Geriatrics, or Palliative Care.
  • Experience working with older adult populations, SNFs, or post-acute care settings.
  • Expertise in data-driven decision-making, healthcare quality metrics, and project evaluation.
  • Knowledge of Age-Friendly Health System principles and geriatric care best practices.
  • Strong interdisciplinary collaboration and stakeholder engagement skills.
  • Proficiency in data reporting, outcome tracking, and quality improvement methodologies.
  • Excellent verbal, written, and presentation skills.
  • Strong analytical and problem-solving abilities.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
