Overview

The Program Manager provides overall leadership and supervision to ensure the quality, safety, and effectiveness of services delivered to persons served. This role oversees daily program operations, directly supervises and develops Rehabilitation Specialists, and serves as a key liaison for families, providers, and community partners. Program Managers are responsible for embedding Recovery Model principles in all practices to promote hope, empowerment, and independence for persons served.

Health Home Responsibilities
  • Collaborate with Health Home providers to ensure integrated care coordination for persons served
  • Report health-related incidents promptly, including falls, injuries, or unexplained bruising, to the Health Home and appropriate supervisory staff
  • Support communication between health providers, families, and staff regarding medical concerns or follow-up care
  • Ensure documentation of all Health Home communications and incident reports meets agency and regulatory requirements
Key Responsibilities
  • Leadership & Staff Oversight
  • Interview, hire, train, and supervise Rehabilitation Specialists, ensuring staff deliver recovery-focused, person-centered care
  • Conduct regular staff supervision, performance evaluations, and professional development planning
  • Review and approve staff timesheets, time-off requests, and mileage; oversee scheduling to ensure adequate coverage
  • Lead team meetings, ensure effective communication of policies, procedures, and agency updates
  • Monitor staff documentation (progress notes, IRPs, incident reports) for accuracy, timeliness, and compliance
  • Provide coaching, mentoring, and direct observation to reinforce best practices and staff accountability
  • Maintain on-call availability to support staff and respond to emergencies
Role: Support to Persons Served
  • Oversee treatment planning, including coordination and review of assessments, IRPs, and interpretive summaries
  • Support clients in pursuing personal goals, practicing life skills, and engaging in community integration
  • Identify early warning signs, manage crises, and promote healthy coping strategies
  • Ensure coordination of psychiatric, somatic, and case management services, including hospitalizations, medication reviews, and discharge planning
  • Maintain communication with family members, providing education on the Recovery Model and updates on care
Program & Operational Oversight
  • Conduct regular housing inspections and coordinate facility repairs, equipment, and supplies
  • Ensure compliance with state, CARF, and agency standards for documentation, safety, and care delivery
  • Participate in intake and referral processes, including interviews and admission/discharge decisions
  • Coordinate with pharmacies, psychiatrists, and other providers to maintain continuity of care
  • Oversee community outings and activities, ensuring they promote social connection and recovery
Responsibilities
  • Case Management & Intake:
    • Ensure all clients are assigned a case manager and support case management in admissions and discharge planning
    • Coordinate financial matters such as entitlement applications with case management
    • Participate in intake interviews and team discussions for new client referrals
  • Housing & Facility Coordination:
    • Oversee housing inspections and coordinate repair needs with the Facilities Director
    • Conduct quarterly inspections of all residential sites to ensure compliance with regulations and professional standards
  • Crisis Intervention:
    • Identify early warning signs of psychiatric and physical decompensation and take appropriate action
    • Diffuse and address behavioral or psychiatric crises to maintain safety and well-being
    • Promote healthy coping mechanisms and assist clients in managing stressors
  • Community Engagement:
    • Supervise Rehabilitation Specialists in facilitating community outings and activities that encourage connections with the broader community
Qualifications
  • Bachelor's degree in a human services field (e.g., Psychology, Social Work, Rehabilitation Counseling, etc.) from an accredited college or university.
  • Minimum of two years' experience working in the mental health or psychosocial rehabilitation field, or an equivalent combination of education and experience.
  • Knowledge and experience with the Recovery Model in mental health and substance abuse recovery services.
  • Strong leadership, organizational, and communication skills.
  • Ability to respond to crises and manage stressful situations.
  • Physical ability to perform office work, visit off-site housing, and assist clients as necessary, including lifting/carrying up to 50 lbs.
Recovery Process Support
  • Maintain familiarity with all relevant client information including diagnosis, treatment plans, and medication
  • Coordinate Individual Recovery Plans (IRPs) and ensure treatment plans are individualized and person-centered
  • Assess clients' stage of change and apply appropriate interventions
  • Participate in the development of treatment plans and overall recovery goals
Coordination of Care
  • Act as a liaison with pharmacies, psychiatrists, and other healthcare providers
  • Ensure timely scheduling of medication reviews and documentation of treatments
  • Coordinate somatic care and hospitalization needs
  • Foster ongoing communication with clients' families, educating them on the Recovery Model
  • Implement crisis intervention and management plans
