We're seeking a Social Worker (MSW) to support the social, emotional, and practical needs of our residents and their families — developing care plans, providing counseling, and serving as a strong advocate for their well-being.

Key Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans
• Provide individual, group, and family counseling
• Advocate for residents' dignity, rights, and appropriate care
• Foster communication between residents, families, and care teams
• Coordinate discharge planning and connect families with community resources
• Provide crisis intervention and end-of-life support
• Educate care teams on culturally competent care
• Maintain accurate documentation and collaborate in care team meetings
• Monitor care plans' effectiveness and contribute to quality improvement

The Ideal Candidate:

• Master's degree in Social Work (MSW)
• Experience in health care or assisted living facility required
• Strong interpersonal and communication skills
• Collaborative team player who advocates for patient dignity
