Base pay range
$38.00/hr - $44.00/hr
Hospice of the East Bay (HEB) is a community-based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses. Hospice East Bay has been certified as a Great Place to Work for the past 5 years in a row! This position serves patients in Oakland, Emeryville, Hercules, Pinole, and surrounding cities.Position SummaryThis position reports to the Clinical Manager. As a hospice and palliative care social worker, responsibilities include providing agency patients and families with assessment, evaluation, counseling and casework to address social, economic, and psychological needs.Essential Function
- Completes initial as well as ongoing assessments of the psychosocial needs of all assigned patients/families.
- Develops plan of care for patient/family that is tailored to individual needs and includes the interdisciplinary team as appropriate.
- Reports patient/family contacts and interventions to other members of the interdisciplinary team appropriately.
- Attends interdisciplinary team meetings, staff meetings, and other meetings as indicated.
- Provides accurate documentation of each contact with or regarding the patient/family in accordance with current regulations.
- Provides interventions as indicated in the interdisciplinary team care plan.
- Completes a suicide assessment and bereavement risk assessment on all patients at the time of the initial psychosocial assessment.
- Completes assessments for: physical abuse, rape, domestic abuse, sexual molestation, exploitation when appropriat
- Reports child abuse/neglect and dependent/elder adult abuse as required by law.
- Provides or arranges for individual or family counseling services to assist with the stress associated with a life threatening illness.
- Identifies and facilitates referrals to appropriate community resources and coordinates patient and family use of these resources.
- Assists with benefit programs and end of life planning.
- Develops and/or participates in education programs and workshops for patient/families, staff and community at large.
- Participates as a proactive and supportive member of the interdisciplinary team.
- Provides input to program coordinator as to the ongoing clinical needs of the program as well as issues of quality improvement.
- Attends and presents appropriate patient-related information at the Interdisciplinary Team Meetings that focuses on open care plan problems.
- Adheres to federal and state regulations, as well as accreditation standards.
- Completes and submits patient-related documentation according to Agency standards.

Position RequirementsMust have excellent interpersonal and communication skills. Understanding of the psychosocial issues related to families in crisis and end-of-life issues. Ability to perform a psychosocial assessment and intervene appropriately. Ability to work effectively with individuals and groups. Ability to work in the home and nursing home setting, as well as in-patient unit. Must be able to handle multiple tasks, be flexible and prioritize work load. Must be highly organized. Ability to provide culturally sensitive support to people from diverse ethnic, cultural, and socioeconomic backgrounds. Must be able to work well under pressure meeting multiple and sometimes competing deadlines. At all times, demonstrate cooperative behavior with colleagues and supervisors.Experience And Education
- Master of Social Work degree from a school accredited by the Council on Social Work Education
- One year social work experience in a health care setting preferred; experience in Long Term Care setting a plus
- Clinical experience with geriatric, AIDS, oncology, or terminally ill populations desirable
- Current CPR certification
- Bilingual (Spanish) skills preferred
