We are seeking Mobile Integrated Health Senior Counselors. The Mobile Integrated Health Program (MIH) is a collaboration between the Seattle Fire Department (SFD) and Aging and Disability Services, a division of the Human Services Department (HSD). Case managers work alongside specially trained firefighters who respond to clients in behavioral, medical or substance use crises. Our case managers are some of the most skilled, experienced and effective case managers in the business. They are true experts in crisis intervention, service connections, and the clients who are referred to the Seattle Fire Department. The ability for our firefighters and case managers to work as a seamless unit is behind everything we have achieved in the program.

The Mobile Integrated Health Program case managers respond to clients and conduct an on-scene assessment and plan of care, referring to services and supports to meet long term needs rather than the emergency room. Health 99 is part of the MIH program. This is our post overdose response team and was created in response to the Mayor's Executive Order to address the fentanyl crisis. Health 99 case managers provide a trauma informed response to clients experiencing a substance use crisis and will follow-up to link clients to treatment or other services.

The Mobile Integrated Health Senior Counselor(s) will help lead this team.The position(s) will serve as lead case managers and support the supervisor and program manager for the Mobile Integrated Health Program (MIHP). The position(s) will assign, prioritize, and monitor the workload of case managers; coordinate and direct case manager training; conduct reviews of case manager documentation, assessments, and care plans; and provide support to staff regarding complex cases. The position will report to a Human Program Supervisor, Senior within Aging and Disability Services/HSD.

When the program expands its hours, this position may transition to some weekend hours. This role will be in the office/field full time with the opportunity to have a hybrid schedule as permitted. This recruitment will be used to fill two openings within the MIH program.
  • Serve as lead staff for MIH Program. Review client referrals and attend staffing daily. Assign clients to the team's case managers. Direct, prioritize and monitor workload. Assist with scheduling of staff and rig coverage.
  • Provide rig coverage on a minimal basis. Provide a trauma informed social work response with SFD fire fighters on rig to clients in medical, behavioral and/or substance use crises. Maintain expert level of skills in all case management functions.
  • Provide training, support and assistance to case managers. Serve as subject matter expert regarding MIH program.
  • Assist with new firefighter training in partnership with MIH leadership and administrative staff.
  • Oversee and lead staff through new service delivery programs and approaches.
  • Participate in meetings with other partner providers for care coordination and program development. Represent the MIH program at community provider meetings as requested.
  • Provide support to supervisor and program manager. Use existing tools or suggest new approaches to ensure accurate and thorough data collection and documentation of key MIH interventions and outcomes · Weekly communications to case management team on scheduling and notable upcoming events, meetings, or outreaches. Assist with coordination of and discussion with high-profile visits or ride-alongs (e.g., elected officials).
  • As the program expands hours, rotate provision of leadership, direction and guidance on the weekends to staff.

Minimum Qualifications:

Four years' social service experience involving interviewing, counseling, crisis intervention, and a bachelor's degree in social services, Psychology, Sociology, or related field (or a combination of education and/or training and/or work experience which provides the ability to perform the work of the class).

Requires a minimum of two years working with older adults, adults with disabilities, people in medical, behavioral or substance use crises. Requires extensive knowledge of services and supports to meet the needs of the Mobile Integrated Health clients.

OR: An equivalent combination of education and experience that demonstrates the ability to perform the position duties.

Certification/License: Driver's license for the ability to make on-site visits to clients and attend meetings and trainings.

Desired Qualifications:

  • Experience serving populations impacted by extreme poverty, behavioral health conditions, substance use and mental illness from a trauma informed and harm reduction model of care.
  • Experience as a Mobile Integrated Health Case Manager.
  • The MIH Case Manager should be an experienced and proficient system navigator, with a working knowledge of medical, behavioral, substance use and social service resources throughout the city, as well as community-based organizations, and strategies for accessing them in real time for clients in need.
  • Strong written and oral communication skills, with demonstrated ability to effectively communicate with a diverse range of clients, providers, policy makers, partners, and staff.
  • Experience providing supervision or directing the work of a professional multidisciplinary team.
  • Experience designing and providing training to adult learners.
  • Strong understanding of race & social justice principals and impacts of systemic racism and oppression on communities served by HSD.
  • Enjoy working in multi-disciplinary teams.

  • The full pay range for this position is $44.29 - $51.62 per hour. Additionally, the Mobile Integrated Health Program Senior Counselors receive $5.00 per hour premium pay.

    This role is expected to be in the office/field a minimum of three days a week. Interviews will be conducted in person at the Seattle Fire Department in Pioneer Square.
