• You must be in the Military Active Duty, Guard, or Reserve Component.
• Maintain appropriate licenses, certificates, and permits necessary or required.

• Proof of immunity to rubella and rubeola (two doses of MMR vaccine or serological testing).
• Proof of varicella immunity (verbal or written history, or titers).
• Hepatitis B series initiation or declination, and/or testing to document serologic response.
• Two-step TB Skin test or Quantiferon testing within the last 12 months.
• Completion of flu shot within flu season (as indicated by state regulatory agency).
• Proof of COVID vaccine or exemption documentation.
• ACLS and/or BLS Certification, if applicable.
• Completion of HIPAA Compliance Training.
• Training in Human Blood and Body Fluid Precautions and Infection Control.
