Job Description

The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs) that may place individuals at risk for HIV infection and complications. Medspan staff support treatment adherence, discharge planning and connection to care services, and they work very closely with infectious disease specialists and other interdisciplinary correctional health staff in all facilities.

Responsibilities:

  • Identify new HIV-positive patients using electronic health records, report manager, and other tracking tools to provide ongoing HIV education, medication adherence support, and re-entry/discharge/linkage to care support
  • Identify new hepatitis C-positive patients using electronic health records, report manager, and other tracking tools to provide hepatitis C education, medication adherence support, and re-entry/discharge/linkage to care support
  • Identify patients known to be living with HIV to support the continuation of antiretroviral medications, medication adherence, recommended screenings and immunizations, harm reduction counseling, and evaluation by HIV specialists
  • Provide education, harm reduction counseling, and medication adherence support for HIV pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) eligible patients
  • Provide education, harm reduction counseling, and medication adherence support for patients diagnosed with STIs or other conditions that place them at risk of HIV acquisition
  • Regularly review antiretroviral medication administration lists and patient labs to identify potential adherence challenges and provide ongoing adherence support and education
  • Regularly review hepatitis C medication administration lists and patient labs to identify potential adherence challenges and provide ongoing adherence support and education
  • Document all direct and indirect patient encounters in the electronic health records utilizing appropriate Medspan templates
  • Obtain HIPAA medical record release forms as necessary to support the continuation of medical care while in the jails and assist re-entry planning and linkage to care in the community
  • Work collaboratively with HIV and infectious disease specialists, discharge planning services, and other correctional health staff across all facilities
  • Regularly complete and update HIV and hepatitis C patient tracking databases
  • Participate in case conferences with the infectious disease service and other re-entry programs
  • Assist with research, analysis, patient tracking, and quality improvement and implementation efforts
  • Attend required trainings and continuing education

Departmental Preferences

  • Passion for health equity and ensuring access to healthcare for underserved communities
  • Experience working with populations living with HIV, hepatitis C, and other infectious diseases
  • Experience working in a jail setting or with justice-involved individuals
  • Experience with care coordination and re-entry planning for individuals experiencing incarceration
  • Ability and desire to work with a diverse group of individuals this includes the ability to communicate respectfully and effectively with patients, clinical staff, and Department of Corrections staff
  • Problem-solving skills, creative thinking, and the ability to self-motivate and work independently
  • A satisfactory equivalent combination of the above with the willingness and ability to learn and acquire new skills
  • Minimum Qualifications

    1. Graduation from a recognized college or university with a major in the biological or social sciences or in education, plus a Master's Degree in public health with major study in public health education, and five (5) years of experience in health education work with a public health or voluntary agency; or

    2. Graduation from a recognized college or university with a major in the biological or social sciences or in education, and seven (7) years of experience in health education work with a public health or voluntary health agency; or

    3. A satisfactory equivalent of training, experience and education.
